Job Qualifications
  • Must hold NYS Master Degree
  • Certification in Special Education and/or School Counselor preferred
  • Must have at least three years of full time experience
  • Must have experience providing Professional Development and/or Technical Assistance related to transition of students with disabilities from school to adulthood 
  • Must have the ability to problem solve, manage projects, research to find answers, and the ability to collaborate well with administrators, teachers and colleagues
  • Must have the experience and knowledge related to transition assessment and planning, adult agencies, work based learning, and culturally responsive transition practices

Coordinator, Transition Services Coordinator                                                                                                  

 

General Duties:

  • The Transition Coordinator will work with district staff, students, and families to provide Transition planning and implementation of services to the students enrolled in the Transition WORKS project. 
  • Become and stay familiar with current research based transition practices,including person centered transition planning; family involvement, use ofcommunity resource; school to career; New York State laws and regulationsregarding topics related to transition; graduation requirements; fundingresources, SSA benefits, VR services.
  • Plan and help provide staff development for participating school district staff andfamilies to ensure that the transition planning and implementation occurs forTransition WORKS participants.
  • Conduct Person Centered Transition Planning or ensure that it occurs for everyproject participant.
  • Participate in case conferencing regarding participants and maintain participantsrecord of participation in the project. Complete and provide requested reports forinclusion in reports to SED and SSA.
  • Assist participants and families in connecting to appropriate community agenciesand supports (i.e., service coordination, counseling) and acts as the liaison withadult agencies for those schools and families in the Project. Ensures all participants have needed services, locating it in the community or by providing ituntil services can be identified and obtained.
  • Attend Transition Works meetings at the local level and other communitymeetings as assigned in the region and state.
  • Act as liaison between school district staff and project staff, families, and friends.
  • Other duties, as assigned, that promote transition outcomes for students andfamilies.
